How to Request Your Prescription, Receipt, Etc.

Prescription Expiration:  Under Indiana law, eyeglass and contact lens prescriptions are valid for a minimum of one year from your exam date. Your prescription is good for one year from your exam date.

Processing Time:  Prescription and document requests are generally processed with 2-3 business days. Please keep in mind, weekends and holidays are not considered “business days”.

What’s Included in Your Prescription:  We provide you with your lens prescription. Your optical retailer is responsible for measuring PD, seg height, and frame fit and adjustments. Note: Indiana and Federal law does not require these measurements to be a part of your eyeglass prescription. Your contact lens prescription will include the brand, prescription, and base curve (if needed).  

Quicker Service:  If you need a document sooner than that, please stop by our office during open hours.
