2. Appointments and Scheduling
2.1 Appointment Booking

2.2 Cancellations and No-Shows
We require at least 24 hours’ notice for appointment cancellations or rescheduling. Failure to provide adequate notice or repeated no-shows may result in charges or restrictions on future appointment booking.
2.3 Late Arrivals
Arriving more than 15 minutes late for your appointment may result in rescheduling to accommodate other patients.

Eyewear and contact lens prescriptions are provided for your use. You may choose to purchase eyewear from Moody Eyes or another provider. Prescriptions are valid for one year from date of exam, as specified by Indiana law. This may be changed based on our professional judgment.
